Welcome to MVC's 2024 Fall Collaborative-Wide Meeting, Data Driven Excellence: Strategies for Success in Quality Improvement! MVC will share updates and announcements from the Coordinating Center followed by a presentation about the MVC Component of the BCBSM P4P Program PY 2026-2027 cycle.

Readmission Reduction: Intelligent Targeting to Timely Intervention

Corewell Health has made notable strides in reducing hospital readmissions. This presentation will offer an overview of the essential recovery behaviors patients must engage in during their transition from inpatient care to baseline health. Emphasizing the importance of addressing whole-person needs in discharge planning and in the real-time identification of complex patients, this presentation will showcase Corewell's recent initiative successes and help participants recognize evidence-based interventions to further improve patient outcomes.

Cardiac Rehabilitation Breakout Session

Munson Healthcare will present a guided tour of their multi-phase cardiac rehabilitation program. Session attendees will learn about the successes and barriers to developing a comprehensive cardiac rehabilitation program in Northern Michigan and review state-wide unblinded cardiac rehab utilization rates based on claims data for MVC collaborating sites. 

Post-Discharge Follow-Up Breakout Session

MVC will guide attendees through a patient journey mapping workshop. Session attendees will collaborate to identify tailored intervention opportunities for patients not currently utilizing post-discharge follow-up care.

Preoperative Testing Breakout Session

This presentation will provide an overview of the importance of increasing appropriateness of preoperative testing for low-risk surgeries, featuring unblinded MVC claims data. Highlighting the new RITE-Size initiative, attendees will learn more about available resources, lessons learned from phase I pilot sites, and opportunities to participate in RITE-Size during phase II. 

Sepsis Breakout Session

Michigan Hospital Medicine Safety Consortium (HMS)

The Michigan Hospital Medicine Safety Consortium (HMS) will delve into the complex impact of sepsis on patients and the motivations behind launching the HMS Sepsis Initiative. Attendees will learn about the HMS sepsis bundles and review performance data related to HMS sepsis discharge measures. Additionally, the session will present unblinded data on 14-day follow-up outcomes after sepsis, utilizing MVC claims data.
