Welcome to MVC's 2026 Spring Collaborative-Wide Meeting - From Innovation to Impact: Advancing Care Across Health Networks. MVC will share updates and announcements from the Coordinating Center followed by a presentation about the MVC Component of the BCBSM P4P Program PY 2026-2027 cycle.

Over the last two decades, there has been a gradual, structural shift in the proportion of hospitals that are independently owned vs health system affiliated. This change has reshaped Michigan’s regional hospital markets and impacts care standardization, quality improvement priorities and community benefit strategies. This presentation will explore strategies for elevating quality across integrated health networks through the lens of system-wide alignment and data-driven improvement.

Building on the Data in Action presentation, this panel discussion will explore what "systemness" and system quality of care mean in practice and how stronger integration can improve quality, outcomes, and access across health systems and networks.

MVC will present engagement and poster session awards and spotlight upcoming meetings and post-event survey information.

Corewell Health’s Catalyst Community for Improvement (CCI) accelerates systemwide performance by unifying teams around a structured, data‑driven improvement model, driving faster outcomes, stronger engagement, and a culture of shared accountability across the organization.

Building a high‑performing post‑acute network is essential to improving patient outcomes and driving operational excellence. Successfully identifying at‑risk populations, deploying targeted interventions, establishing consistent standards of care, and ensuring reliable handoffs across different care settings require intentional alignment with high‑quality post‑acute partners. Our new statewide programs bring this alignment to life by standardizing care transitions, strengthening coordination between inpatient and post‑acute teams and enabling seamless, patient‑centered care across the continuum.

As we prepare for value-based reimbursement in the primary care payor-realm, the heart failure population is expected to rise to 8.5 million Americans and preparing is vital. Effective care management is crucial for enhancing the management of this population by implementing cost-effective strategies that enhance quality and improve outcomes.

Join MVC to discuss claims data strategies for rural and critical access hospitals. We will dive into previous push reports and custom reports that have been valuable to our members’ QI work, and we will have engaging discussion on how claims data can be used going into the future.

This interactive breakout session will introduce MVC’s goals and vision for developing future system-level metrics to support health system-wide quality improvement. Participants will hear about several early metric concepts under consideration and will have the opportunity to provide feedback on their relevance, feasibility, and potential value in driving system-level change. The session is designed for system-level quality and clinical leaders who are interested in helping shape the development of meaningful new metrics.
